WAIVER OF LIABILITY AND DECLARATION
I, for myself, my heirs, my executors and administrators, do hereby waive and release any and all rights, claims and causes of action I have or may have against streetballerHK Basketball, and all sponsors, promoters, supporters and all other contributors from any and all liability arising from illness, injury, death, loss and economic consequences I may suffer as a result of my participation in StreetballerHK 青少年籃球聯賽 from any cause whatsoever, including negligence. I certify that I am physically fit to participate in this event. Further, I grant permission to the organiser, Sponsor or their agents to use any photograph, video or any other record of the event for any lawful purpose.

豁免法律責任及聲明
本人、本人之繼承人、本人之遺囑執行人及本人之管理人謹此豁免 streetballerHK Basketball、所有贊助商、支持是項運動之團體及任何有關之團體對本人因參加是項籃球賽而由於任何原因，包括疏忽，所引致之疾病、死亡、個人損失及經濟損失之任何法律責任，以及放棄任何有關之權利、索償及追究行動。本人同時亦在此聲明本人身體狀況良好適宜參與本賽事。此外，本人亦授權予舉辦者、贊助商及其代理商選用任何有關此項賽事之相片、錄影帶及賽事紀錄作任何合法用途。
